Special Session for Volunteers of Hospice Buffalo: It’s Never Too Early for Advance Care Planning

While we often plan for milestones like careers, families, and retirement, many people avoid thinking about the healthcare they would want to receive if their health status changed and they were unable to speak for themselves.

Many avoid thinking about it and even more avoid talking about it. Yet doing so might be the greatest gift you can give to yourself, your loved ones, and even your healthcare team - advance care planning helps ensure your wishes are known and respected when you can’t speak for yourself.

Join us to learn more about the questions you should ask yourself, the conversations to have with loved ones and healthcare providers, and the documents that will guide your care including:

  • Healthcare Proxy

  • Non-hospital Do Not Resuscitate (DNR)

  • Medical Orders for Life-Sustaining Treatment (the bright pink MOLST form)
